Scheduling An Appointment

To request a consultation/appointment, please complete this form below or call our office. You will receive a confirmation after you have submitted this form for your own records.

​If we have available sessions with providers, we will call and/or email you. If not, you will not receive a call or email and rather be added to our waitlist. You will receive a call in the future when we have availability to schedule you. You are always welcome to call our office to get an updated wait time if we are able to provide an estimation. Remember, these estimated wait times are a best guess and often changing depending on current client needs. 

If you have concerns related to body image, disordered eating, nutrition, and related concerns. please follow this link to complete an additional form to help us find staff that best fits your needs.​ After reviewing the information, we may recommend that clients with eating concerns work alongside a dietitian at the same time as their work with an eating disorder trained counselor because the work you are doing with one provider complements the work you are doing with the other provider and supports more efficient successes. This recommendation also supports our holistic care approach. For instance, if your dietitian recommends a meal plan or food exposure that may feel daunting, your counselor will be able to help you process possible distress and build the coping strategies necessary to follow through on this goal. By waiting to be able to start services with both providers at the same time, it ensures you will have the proper level of support needed to be successful.

All clients or parent/guardian of minor aged clients are required to complete all intake paperwork as follows: Monday or Tuesday appointments paperwork needs to be completed the previous Friday before noon (end of our business day) and for Wednesday through Friday appts 48 hours before your intake appointment, otherwise we will proceed with cancelling the appointment until the paperwork is complete. This information is immediately provided to you when scheduling as well. 

A parent/guardian is required to attend the intake session with a client under the age of 18. 

Insurance & Payment Guidance

​If your insurance is not listed in the question below about insurance, then we are considered out of network and are able to provide you with a "superbill" to submit directly to your insurance. Please ALWAYS verify that your health plan includes dietary benefits as well if seeking those services. Some plans have many exclusions defining requirements for use of those benefits. Please read the providers bios to verify what insurance they accept as each may accept different forms of payment. ​Please be aware we do not take Badgercare/Medicaid/Forward Health or Medicare. ​Synergos requires a major credit card on file in order to schedule appointments, and that I can also add an HSA/FSA card and ask that the major credit card is used as a backup or for missed sessions/late cancellations. Missed sessions/late cancellations are charged at the rate of $100. There may be extenuating circumstances contributing to a late cancel or missed session, so please discuss directly with your provider.

Confidentiality

We are offering HIPAA compliant telehealth services as well as some limited in-person options. If using telehealth, please check with your insurance to assure you have telehealth benefits. Additionally, we require that you have access to both video and audio features in a secure location when participating in telehealth as this is most often required by your insurance for reimbursement. We are unable to do sessions while you are in your vehicle traveling.

All staff at Synergos Counseling also participate in regular consultation groups in which they may seek support or feedback regarding how to best meet the needs of a client or to identify resources that may benefit a client. We encourage all clients to be familiar with who all of the Synergos providers are and inform your provider if you have any type of personal or professional relationship or conflict of interest with any other Synergos Counseling team member.
